The checklist helps a reviewer spot mismatches before another file is submitted. It does not determine legal status or guarantee marketplace acceptance.
Record the exact prompt, affected account or product, deadline, and rejection reason if one exists.
Match the application role—such as manufacturer, importer, repacker, or reseller—to the evidence path in the live request.
Identify whether the request calls for a portal record, listing identifier, label, invoice, exemption statement, or another source record.
Compare legal names, facilities, addresses, products, and seller-account details across the request and evidence.
Keep facility registration numbers, FEIs, Cosmetic Product Listing Numbers, and other identifiers distinct.
Verify dates, current status, legibility, completeness, and the context needed to understand a screenshot or document.
Save the exact outward-facing files, evidence index, submission date, confirmation, decision, and next action.
The checklist is useful when a marketplace asks for an “FDA certificate,” registration evidence, a product-listing identifier, an exemption statement, an invoice, or supporting label records. FDA says it does not issue certificates or other documents verifying cosmetic facility registration, product listing, or exemption status, so the response should be built from the records that actually exist.
