Reporting note

Employers should use current IRS forms, instructions and payroll-provider guidance before filing. This guide focuses on internal reporting support and reconciliation.

W-2 Code TA reporting depends on clean contribution records. Employers should be able to trace the amount reported for each employee to approved contribution cycles, funding files, trustee confirmations and corrections.

What to preserve for W-2 Code TA support

  • Employee identifier, name and tax year.
  • Contribution cycle and payment date.
  • Beneficiary and account-verification support.
  • Amount approved, amount funded and amount accepted by the trustee.
  • Returned, rejected, reversed or corrected payments.
  • Annual employee total for Section 128 employer contributions.
  • Reconciliation to payroll reports, trustee confirmations and the general ledger.

Reconcile before year-end close

Do not wait until W-2 preparation to investigate differences. A monthly or quarterly reconciliation can identify rejected payments, duplicate records, stale certifications and contribution amounts that do not match the written-program formula.

Watch for employee-level aggregation

When contributions are made for dependents, payroll still needs to support employee-level reporting and limit controls. Store beneficiary detail separately from the employee annual total so both can be reviewed.

Build a correction trail

If an amount is later determined not to qualify as a Section 128 contribution, the employer should retain the analysis, notification, trustee response, payroll adjustment and reporting treatment. Corrections are part of the year-end evidence file.
